At Mercy, we want you to feel your best and to be confident in managing your own care at a level that’s comfortable for you. Mercy Patient Centered Medical Homes (PCMH) are staffed by experienced primary care physicians and other health providers who will partner with you on these important goals while providing quality, compassionate care that’s safe, comprehensive, understandable, and accessible.
Similar to a traditional family or internal medicine practice, a Mercy PCMH offers the day-to-day health care you and your loved ones need. However, at a PCMH, we take primary care a step further by inviting you to play an active role in your own and your family’s health care. As part of this team approach, we will:
- Communicate and share information in way that makes sense to you.
- Help you learn what you need to know in order to manage your own health.
- Include you in decision making about your health and the health of your family.
- Share with you the latest data about evidence-based care; medical research; and our quality and safety initiatives.

There are many benefits to choosing a Mercy PCMH for your care. As your partner in health, we take our responsibilities to you and your loved ones seriously.
At a PCMH, you will receive comprehensive care, regardless of insurance type. That means our team will strive to meet a majority of your physical and behavioral health care needs with the appropriate care at the right place and at the right time. We will:
- Listen to your concerns
- Answer your questions
- Ask for your input
- Offer education to allow you to take an active role in your own health and the health of your family members
If you are currently uninsured, we can give you a Medicaid hotline number and assistance with the application process.
We will collaborate with other doctors and health-care professionals to coordinate your care, paying special attention to transition periods. Our goal is clear communication with you and with all your health-care providers, including:
- Medical specialists
- Behavioral health providers
- Rehabilitation facilities
- Community services
If you need behavioral health care, we can refer you to a variety of behavioral health professionals in the area, including community-based facilities for the uninsured. Many companies offer an Employee Assistance Program (EAP), which provides free behavioral health services to their employees.
At a PCMH, we offer convenient office hours, including extended hours on certain days. We will also increase your access to care by offering same-day appointments. After-hours calls are automatically directed to our Clinical Communications Center with access to registered nurses who will listen to your concerns and provide clinical advice.

We are committed to offering safe, quality care for you and your family. We are also committed to quality improvement, which includes getting your feedback about your patient experience and satisfaction. We want your feedback so we can continue to improve your quality of care.
Take an Active Role in Your Own Health Care
When you partner with a Mercy PCMH, you play a significant part in making sure you get the very best care. That includes:
- Telling us your medical history, symptoms and other important information. This may include past illnesses and surgeries, family medical history, medications you are taking, etc.
- Calling us first with any health concerns, unless it’s an emergency. If you are not sure what counts as an emergency, ask your doctor or another PCMH provider.
- Letting us know if you see another doctor, such as a specialist, or if you have been admitted to the hospital. We want to follow up on your care. When you see another health-care provider of any kind (specialist, urgent care physician, hospital, etc.), let them know about your PCHM and who your doctor is, so they can follow up with us, as well.
- Ask us questions and work with us to set goals. Share any concerns you have in reaching those goals, so we can figure out the best plan for success. Then, let us know how you’re doing.
Your Personal Health Information
Mercy HEALTHeChart online portal gives you access to your personal health information 24/7. This patient portal is a secure, web-based tool for providing free, secure access to health information and online communication with Mercy staff.
All of the information in your HEALTHeChart Patient Health Portal comes directly from your Mercy medical record. This ensures that you have access to the most accurate, up-to-date information possible.
HEALTHeChart allows you to:
- Print Information to share with your providers
- View laboratory and radiology results
- Review your list of homegoing medications & allergies if you’ve been an inpatient
- Pay your bills online
- View upcoming appointments and track visit history
- Update your personal information

#1: Have your questions ready.
There may not be enough time to answer all of your questions in one appointment. Please highlight and ask the most important questions first. Be sure to keep track of the answers.
#2: Find out what health problems come first.
Talk with your PCMH team to decide which health problems to work on first.
#3: Repeat back your health plan.
In your own words, repeat back your health plan to your PCMH doctor or health-care provider. This will ensure you and your care team understand each other and that you know everything you need to do before your next appointment.
#4: Get after-hours information.
Ask a PCMH team member the best way to reach them after hours.
#5: Transfer medical records.
If you need to transfer your medical records to the practice, the PCMH office staff will provide you with a release-of-information form.
