Our office is available from 8:00 am-4:30 pm and may be reached to schedule an appointment at 330-489-1362. A medical provider is available “after hours” by calling our office phone number and speaking with one of our operators. At this time, an operator will document your concerns, contact the provider and someone will follow up with you. If you need an appointment, prescription refill or test results, please call during regular business hours.
Although our practice does not offer “walk in “appointments, we have multiple STATCARE (urgent care) locations close by that will treat your immediate needs should we not be available.
As a NEW patient, you must arrive 30 minutes prior to your scheduled appointment time. This will allow you time to complete necessary paperwork and it also provides time for our team to enter your patient and insurance information into our electronic medical record. Established patients are required to arrive 15 minutes before the scheduled appointment time. Please be sure to bring your insurance card, any copay that applies and a form of identification to every visit. Patients 17 years of age and under must be accompanied by parent, guardian or present a signed authorization to diagnose and treat.
Late Arrival and Cancellation of Appointments
Please arrive promptly for all scheduled appointments; however, on rare occasion, you may be late to a scheduled appointment. Should you arrive more than 15 minutes after your scheduled time, your appointment may be rescheduled. Please be courteous and call promptly if you are unable to attend your appointment. If it is necessary to cancel your scheduled appointment, we request you call at least 24 hours in advance. This gives the office the opportunity to schedule other patients who are in need of care. The term “no show” is referenced when a patient misses an appointment without prior notice. Unfortunately, ‘no show” appointments are an inconvenience to those patients who need access to medical care in a timely manner. Three “no show” appointments in a 2 year period of time, may lead to dismissal from the practice.
Insurance and Payments
The patient or patient’s guarantor is solely responsible for all insurance matters or payments due to this office. Patients are responsible for co-pays at time of service. If applicable, you will be billed for services not covered by your insurance (as stated in your insurance contract) by our billing department. Self-pay patients are expected to pay the physician’s charge on the day of the appointment and are eligible for a prompt pay discount. When appropriate, payment arrangements can be made through our billing company. Payments can be received at the office for any outstanding balances.
Completion of Forms/Letters
We understand that at times, various forms and letters may be required to assist you with your healthcare needs. Staff will be happy to complete forms and write medical letters as necessary upon your request. However, this is time consuming and accuracy is of upmost importance. Please allow 24-48 hours for completion of requested forms/letters. For forms more than one page in length there will be a $30.00 charge that must be paid prior to the form/letter being released. Insurance companies will not be billed for this service.
Workman’s Compensation Injuries and Motor Vehicle Accidents
Workman’s Compensation Injuries are not treated at our primary care practices; however, a provider is available at Mercy Work, Health, and Safety in North Canton. Services rendered due to a motor vehicle accident are the financial responsibility of the patient
Prescription Refills and Pharmacy Information
Please inform our office of your pharmacy of choice and update us should this change. We encourage patients to review their medications prior to their office appointments and request refills at that time. Please bring a list of your medications to every visit to assist us in better coordinating your care. Please allow 24 hours to refill all medications.
We do not refill prescription after office hours or on the weekends. All controlled substances will be picked up at the office and proper identification must be presented. Mercy’s Primary Care Provider’s do not offer chronic pain management and will not dispense chronic pain medication. We will provide you with a referral to a pain management center if it is determined you require the specialized from of care.

Mercy Professional Care Physicians has introduced online access to your personal health record through HEALTHeChart. You can gain timely access to information and resources to make informed decisions about your health. Through this site you can send messages to your physicians, read about health conditions, review test results, renew medications, update contact and demographic information and even schedule appointments.
