Homecare Telemonitoring

Mercy Homecare Telemonitoring

TelemonitorChronic diseases such as heart failure, diabetes and chronic obstructive pulmonary disorder are on the rise and becoming increasing complex to manage. Mercy Medical Center Homecare & Hospice has a solution: telemonitoring, which allows nurses to collect patients' vital signs – including weight, blood pressure, heart rate, temperature and oxygen saturation – via a telephone modem. Easy to use, the monitor verbally and visually prompts patients to answer a few questions and also serves as a medication reminder. With telemonitoring, homecare clinicians can monitor patients in between visits giving patients and their families a sense of security.
