What is HEALTHeChart?

HEALTHeChart is a secure online tool that connects you to your personal health record. After you enroll, you can connect to it through Mercy Medical Center’s website: www.cantonmercy.org/healthechart.
 

What information can I see through HEALTHeChart?

HEALTHeChart allows you to see your personal health record. It also allows you to view or print: selected appointments, most test results and imaging reports, visit history, discharge medication lists or your continuity of care documents.
*Please be aware that information about your each hospital visit will not be posted to HEALTHeChart until after discharge. Also, HEALTHeChart is NOT intended for communication of medical or emergency issues. Patients should continue to communicate directly with physicians regarding care.
 

How do I sign up for HEALTHeChart right now?

There are a few convenient ways to establish your personal HEALTHeChart account right now:
  • Provide the required information at Admissions.  You will receive an informational letter within your admissions packet.  Complete the enrollment by following the instructions in your email.
  • Provide the required information to your Scheduler.  Then complete the enrollment by following the instructions in your email.
  • Go to the Medical Records department in person and ask to enroll in HEALTHeChart.
 

What if I want to enroll in HEALTHeChart at a later date?

We encourage you to enroll now, but there are also a few convenient ways to establish your personal HEALTHeChart account at a later date:
  • Provide your email at the time of admission. You will automatically receive an email invitation to sign up to the HEALTHeChart patient portal.
  • Call 330.489.1438 and ask to enroll in HEALTHeChart.
  • Email HEALTHeChart@cantonmercy.org and ask to be enrolled in the HEALTHeChart patient portal.
 

What information do I need to enroll in HEALTHeChart?

You will need your contact information. Also, no matter how you enroll, you will need a valid personal email address.
 

Do I have to provide my email address?

Yes, an email address is required for setting up your HEALTHeChart account. If you do not have an email address, you can create one for free through Gmail, Yahoo, Hotmail, or any other email service provider.
 

Why do you need my email address?

Your email address will be your username for HEALTHeChart, thus it is required. Also, it is through that email address that you will receive notification to log on to your secure HEALTHeChart account whenever new information has been made available.
 

How will I know my HEALTHeChart enrollment was successful?

Once you have completed the enrollment paperwork, you will receive an email from the hospital. The message will contain a temporary user ID and password. Please use the temporary user ID, password and link provided to log in to HEALTHeChart. Please note that the enrollment email invitation is only active for 14 days from receipt.
 

Do I need any special computer equipment to use HEALTHeChart?

No. All you need is access to a computer, an email account and an Internet connection.
 

Who can access my HEALTHeChart?

Only you – or a person of your choosing – can access your HEALTHeChart. HEALTHeChart can be used by legal guardians or caregivers with durable power of attorney for health care of dependent adults to access portions of the medical record of their loved ones. A HEALTHeChart proxy form is available to allow parents or legal guardians access to a patient’s online medical record.
 

How do I grant access to someone else for my HEALTHeChart?

Please visit the Medical Records department with the patient and complete the HEALTHeChart proxy form.
 

Can I set up a HEALTHeChart account if I am not yet 18 years old?

Patients must be 18 years or older to sign up for a HEALTHeChart account.  If you are under 18 years of age, your parent or legal guardian will need to establish a connection to you via proxy access.  Please visit the medical records department with your parent or legal guardian.  Once you turn 18, you will automatically disconnect from your parent or legal guardian’s account and you may sign up for your own account.
 

Can I email my physician through HEALTHeChart?

HEALTHeChart is not an email system and we do not recommend sending any confidential information through standard Internet email. When you activate your personal HEALTHeChart account, you will be asked to provide your email address so that you can receive email notification to log on to your secure HEALTHeChart account whenever new information has been made available. Any confidential patient information transmitted from your Mercy health care team will be located in secure messages that you will see only after you have logged into your secure HEALTHeChart account.
 

Will my Mercy Medical Center physician contact me through HEALTHeChart?

When your physician releases selected test results, a message may be included with your results to help you better understand the significance of the information you receive.  However, HEALTHeChart is not an email system and it is not intended for communication with your physician. You should continue to contact your doctor as you always have.
 

Does every Mercy Medical Center patient automatically have HEALTHeChart?

No. Though we strongly encourage you to sign up for HEALTHeChart so that you may benefit from its many convenient uses, a HEALTHeChart account will only be established for you at your specific request. We respect your personal preference regarding communication with your Mercy physician, but we do not require anyone to become a member of the HEALTHeChart community.
 

Is there a monthly charge for HEALTHeChart?

There is no charge. HEALTHeChart is offered complimentary to Mercy Medical Center patients.
 

If I have trouble accessing my HEALTHeChart account, who should I contact?

For help, please contact the HEALTHeChart Information Line at 330-489-1438 or email HEALTHeChart@cantonmercy.org and someone will respond back to you within two (2) business days.
 

If I have questions about HEALTHeChart, who can I reach out to?

We welcome you to reach the HEALTHeChart team at HEALTHeChart@cantonmercy.org or to call the HEALTHeChart information line at 330-489-1438 and someone will respond back to you within two (2) business days.
 

How can I establish access to a HEALTHeChart account of my child/spouse/parent(s)/significant other?

Both the patient and the proxy will need to review and complete a HEALTHeChart proxy access form. You should be aware that a proxy will have the same online access to the patient’s personal health information.
 

My child turned 18 and I am no longer able to access his/her HEALTHeChart account. How do I reestablish access?

You and your child will need to visit the medical records department together and complete the HEALTHeChart proxy access form.
 

I no longer wish to have proxy access to another patient’s HEALTHeChart account. How do I turn it off?

If you have the medical identification number of the patient for whom you are the proxy, staff here at the Medical Records department can turn off your proxy access to the HEALTHeChart account.
 

I no longer wish to have access to HEALTHeChart. How can I close my account?

Please provide your state-issued photo ID, and staff here at the Medical Records department can disable your HEALTHeChart account.
 

Some of my information is wrong. How do I correct this?

If you notice an error in your personal information, such as address and phone number, or if your medical history is incorrect, please contact the HEALTHeChart Information Line at 330-489-1438 or email HEALTHeChart@cantonmercy.org and give us your correct information.

How do see my test results?

Your test results and reports are both available by clicking on the Health Record button on the portal home page. Once you click on the Health Record button you will be taken to the Health Record page where you will see a button on the right labeled Results where you will have access to your Laboratory Results and a button labeled Reports where you will have access to general radiology reports. Laboratory results are available 36 hours after it is resulted.  Radiology reports are available 36 hours after they have been dictated.
 

I don't have an email address, how do I get one?

There are many free email services available to use. Any email service is compatible with My HEALTHeChart. If you are looking to create a free email account, some of the more established email providers are:

Yahoo! Mail from Yahoo (http://login.yahoo.com )
